To prepare for your consultation please follow these 2 steps  . . .

Once completed please fax the Patient Intake Form to 1 - 403 - 237 - 0747

The Virtual Naturopath Intake Form gives the doctor a tremendous amount of information that will become the basis of conversation during the Initial Consultation. Given this in-depth overview of your current health state and health history, the doctor will be able to ascertain the necessary treatment plan or test required and oversee your next steps.

Once we are in receipt of your Waiver and Patient Intake Form, our office manager Nicole will contact you to arrange the time for your Initial Consultation.
